HART Down Payment Assistance Procedure for Agencies

Lenders/Brokers:
Please submit the following to HART Processing Department, fax# 909/941-4012:
   √ Request for Gift Funds (Submit online or fax to HART)
   √ HART Gift Letter
   √ Seller Participation Agreement
   √ Copy of buyer’s “Purchase Agreement”, and additional instruction for escrow,
         please refer to “Sample Page”
   √ If HART fees paid by Lenders/Brokers, “Additional Instruction for
         Lender/Realtor/Broker Paying the HART FEE”

Escrow/Closing Agencies:
Please submit the following to HART Processing Department, fax# 909/484-1298:
   √ Copy of “Settlement Statement/HUD-1”
   √ Wiring Instruction
   √ Closing Application
   √ Additional Instruction for Lender/Realtor/Broker Paying the HART FEE
          requires to be signed on the bottom
